Charges – The cost of your evaluation and treatment will depend on your medical problems.  It is not possible to give you an exact cost prior to your appointment, but we can give you a range.  We make every effort to treat you and your family with the best possible care in cost effective manner.  It is customary to pay for your medical care at the time it is given.  We will help you file your insurance but the bill is ultimately the patient’s responsibility.  You will receive a statement if there is a balance due on your account.  Please review it carefully and pay it by the due date.  We can accept cash, check, Visa, MasterCard or Discover.  Returned checks will be assessed a service charge of twenty-five dollars.  If paying your balance in full is not possible, we can set up a payment plan.

Phone calls - Routine phone calls should be made during office hours.  This includes all prescription refills.  When calling for a prescription refill please have available the name and dosage of the medicine and the name and phone number of your pharmacy.  Please call during office hours for non-urgent questions and chronic problems.

After Hours Care – Dr. Nish, Dr. Shanks or Susan Brown, or another qualified physician will be available by phone after regular office hours for medical problems which cannot wait until office hours.  If you have an urgent problem after hours call (770) 535-3553 and ask for Dr. Nish.  You should expect to be called back within 30 minutes.  If you have any emergency and cannot wait to be called back, call 911 or go to the nearest emergency room.

Changes – Please keep us informed of any changes in your address, phone numbers or insurance information.  This allows us to provide you with the best care.
